1. Who this is for
- Wisconsin nurses and other DSPS-licensed clinicians who changed their legal name (usually after marriage).
- People who must update more than one place: state license, NPI, DEA, Medicare/Medicaid, cert boards, employer.
- Staff RNs with a short list, and APRNs with a long list.
2. What “done” means
- Legal ID matches the new name (Social Security card + Wisconsin driver license or state ID).
- Wisconsin DSPS license shows the new name.
- NPI record shows the new name (number stays the same).
- DEA shows the new name, if you have a DEA number.
- Medicare / Wisconsin Medicaid / CAQH updated if you bill or credential through them.
- National certifications updated if you hold them.
- Employer HR, badge, and privileging files updated.
- Public checks (license lookup, Nursys, NPI Registry) show the new name.
3. Order of steps (do not skip the order)
- Get a certified marriage certificate (or other legal name-change proof).
- Update Social Security (name on card changes; SSN digits do not).
- Wait at least 48 hours.
- Update Wisconsin DMV driver license or ID (in person).
- Update Wisconsin DSPS LicensE (name amendment; within 30 days of the legal change).
- Optional: order a new DSPS wall certificate (Form 3082, mail, fee).
- Update NPI in NPPES.
- If you prescribe: update DEA.
- If you bill Medicare: update PECOS (after SSA + NPI).
- If enrolled in Wisconsin Medicaid: update ForwardHealth demographics.
- If you use CAQH: update and re-attest.
- Update each national cert board you hold (ANCC, AANPCB, NBCRNA, etc.).
- Nursys follows the Board of Nursing record — you do not edit Nursys yourself.
- Notify employer, medical staff office, malpractice carrier, and related accounts.
4. What you must do yourself
- SSA name change (often appointment or online path; identity proof required).
- DMV visit (bring proof; surrender old card; pay duplicate fee).
- Log into portals that require your password and MFA (LicensE, NPPES, DEA, PECOS, ForwardHealth, CAQH, cert portals).
- Upload or show government ID and marriage proof when a site asks.
- Confirm with your employer what their credentialing office needs.
5. What this product is built to do
- Ask what licenses and numbers you hold.
- Build a personal checklist in the correct order.
- Show deadlines (example: DSPS 30-day notice rule).
- Fill and prepare forms that can be mailed.
- Send mail or email packets to agencies that accept them.
- Give step-by-step instructions for each portal.
- Track what was sent and what is still waiting.
- Check public lookups until the new name appears.
- Stop and wait when a later step depends on an earlier one.
6. Get Started flow (wizard — to be built)
- Create account / start case.
- Enter prior legal name and new legal name.
- Enter date the legal name change took effect.
- Select credentials you hold (RN, LPN, APNP, PA, etc.).
- Answer yes/no: NPI, DEA, Medicare, ForwardHealth, CAQH, which cert boards.
- Upload scans: marriage certificate, SSN card (when ready), new DL (when ready).
- Review the generated task list.
- Approve each mail or email before it goes out.
- Mark in-person and portal steps complete when you finish them.
- Watch verification status until each required system shows the new name.
7. Channels we use
- Mail: forms and letters (examples: Vital Records F-05281, DSPS Form 3082, paper Medicare/NPI forms, some cert boards).
- Email: packets with attachments (example: ANCC name-change request).
- Guided portal: checklists for LicensE, NPPES, DEA, PECOS, ForwardHealth, CAQH.
- In-person packet: SSA and DMV prep only.
- Verify: public license lookup, NPI Registry, Nursys display.
8. What we do not claim
- We do not replace a lawyer.
- We do not guarantee board processing times.
- We do not silently submit to government sites without your approval.
- We do not finish SSA or DMV for you without you showing up when required.
- We do not edit Nursys; the board record drives it.
9. Simple path vs full path
- Simple (many staff RNs):
- Marriage proof → SSA → DMV → DSPS LicensE → employer.
- Full (many APRNs):
- Everything in the simple path, plus NPI, DEA, Medicare, Medicaid, CAQH, cert boards, privileging, malpractice.
- The wizard chooses the list from what you say you hold.
10. After the name is live
- Download or print the updated DSPS credential if you need a copy.
- Keep copies of confirmations and tracking numbers.
- Tell payroll / badge / EHR if the employer did not already update them.
- Update remaining personal accounts (bank, associations) as needed.
