Service fee to prepare, mail/email, and track your name-change paperwork.

*Filing fees are billed at cost. Depending on what you select, that may include:

  • DSPS Form 3082 wall certificate: $10 if you request one
  • ANCC duplicate wall certificate: board fee (about $40) if you request one
  • Extra certified marriage certificate copies (county / Vital Records)
  • Certified or tracked USPS to boards that require paper
  • Any other agency fee we must pay on your behalf — at cost

  1. Certified marriage certificate (or other legal name-change proof)

    Raised seal / official certified copy — not a plain photocopy.

  2. Social Security name updated

    New Social Security card ordered or received. SSN digits stay the same.

  3. Waited at least 48 hours after SSA

    Wisconsin DMV needs time to verify your new name.

  4. Wisconsin driver license or state ID updated

    In-person DMV visit done; new name is on your DL/ID.
